Field Trips at Kamaile

The links below will take you to electronic versions of all of the forms that you need to complete and submit as noted. Please contact your Associate Principal or the Navigators' Center if you have any questions. 
 


CHECKLIST
It is YOUR responsibility to ensure that all requirements are fulfilled in planning your field trip. Use this list to track that you have completed all steps. 

 

 
FORM 1: REQUEST FOR FIELD TRIP
Submit original to your Administration for approval by FIVE WEEKS prior to field trip. Upon review by Principal a copy will be returned to the requesting teacher. Original returned to Administration to be added to the master calendar by Associate Principal 1 (Anna) then forwarded to the Navigators' Center to keep on file.  Requesting teacher should work with Business Manager (Janet) to coordinate funding and purchase order completion portion of field trip.

 

 
FORM 2A: FIELD TRIP NOTICE TO PARENTS (normal field trip)
FORM 2B: FIELD TRIP NOTICE TO PARENTS (overnight or off-island)
SUPPLEMENT: USE OF PRIVATE VEHICLE FORM (for use of private vehicle to transport students)
Type or print all pertinent information and reproduce as needed. Communicate with Special Education teacher regarding passing out and collecting field trip forms for Special Education students.
CHECK RETURNED NOTICE FOR THE FOLLOWING:
a. Parent authorization signatures.
b. Medical coverage/non coverage.
c. Teacher to keep permission form in case of emergency and take with them on the scheduled field trip.
 
 
FORM 3: LUNCH COUNT FORM (4 weeks prior to event)
FORM 3a: FINAL LUNCH COUNT FORM (1 week prior to event)
SACK LUNCHES ARE PROVIDED FOR SCHOOL DAY FIELD TRIPS! Lunch count is still required so that the cafeteria has time to make the necessary preparations. Pre-count due 4 weeks prior to event. Final count due 1 week prior to event. Submit both forms to the Front Office Clerk. All lunch arrangements must process through Front Office Clerk. (Do not go through Café manager.) Teachers are responsible to provide coolers  the day before field trip and pick up lunches in the morning. If lunch is not requested for your trip, please complete lunch request form and indicate (No Lunches Required) on the form.
 
 
FORM 4: REQUEST FOR PARK USE
If you will be using a City and County park for your field trip please complete this form and mail to the address listed.
 
 
FORM 5: STUDENT ATTENDANCE
Complete for all students staying back or absent on the day of field trip. Turn in to the eSIS office before you leave.
 
 
FORM 6: KAMAILE SCHOOL BUS
Complete for use of Kamaile school bus. Submit form to Front Office Clerk for submittal of approval from Paul Kepka. Form must be submitted 4 weeks prior to field trip to allow ample time for scheduling and approval. Original to be filed with AP 2 and a copy provided to requestor.  Upon bus approval, field trip information will be added to the BUS SCHEDULE calendar and the requestor(s) will be invited to the event.
 
 
If your trip requires more capacity than one(1) Kamaile bus can accommodate, please arrange transportation with a private company. Teachers are responsible to make arrangements.  Here are some companies you can try:

FIELD TRIP REMINDERS (for the day of the trip)
 
 
 
NOTE: FIELD TRIP COLLECTION FORM
Is to be filled out by each classroom teacher and must be turned in on the day that money is collected. Collection form is located in the front office. Collection envelope must be turned in by 12:00pm daily to the front office. Envelopes will be returned the next day. All monies must be turned in five school days prior to field trip. NO EXCEPTIONS.
